You may submit a Waiver instead of a completed Emergency Form only if you correct your Emergency Information and Contact Information on line before classes begin. When you sign the Emergency Form or the Waiver, your signature represents your agreement to comply with the CBI Behavior Guidelines for Students. Please read the Behavior Guidelines before registering for classes.
If your student is seven years old or older, please discuss the meaning of the Behavior Guidelines with him or her. We ask students age seven or older to sign under their parent/guardians' signatures to indicate their understanding and agreement as well.
Thank you for helping us keep our records up-to-date and for helping build a student community on foundations of safety as well as kavod, respect and honor.
